William Harry Bruce Chapman, aged 24, died on Saturday 10 October 2026 after scaling safety fences at Yorkshire Wildlife Park near Doncaster.

Opening the hearing at Doncaster Coroner’s Court, coroner’s officer Caitlin Brookes outlined the initial findings surrounding the sequence of events.

As reported by Joe Middleton of The Independent, Ms Brookes stated that:

“He was approached by the tiger and the ensuing physical contact between the tiger and Mr Chapman led to him sustaining multiple traumatic injuries which rapidly led to his death.”

The court heard that Mr Chapman arrived at Yorkshire Wildlife Park at approximately 1:30 pm on 10 October.

He proceeded to climb over two boundary fences before entering the Amur tiger enclosure.

Wildlife park staff acted quickly to secure the animal, and emergency paramedics attended the scene shortly afterwards.

Paramedics formally confirmed Mr Chapman’s death at the scene due to the severity of his injuries.

An initial post-mortem examination concluded that the cause of death was multiple traumatic injuries resulting from the encounter.

What Did the Inquest Hear Regarding Mental Health Services?

The court was told that Mr Chapman, born in Chichester in 2002, was living in Doncaster and working as a self-employed online trader.

He was under the care of adult mental health services in Doncaster at the time of his death.

However, the inquest heard he was considered “highly functioning and independent with no care requirements”.

Ms Brookes confirmed that Mr Chapman’s family had raised “a number of concerns” regarding his contact with mental health providers before his death.

In light of these concerns, the Rotherham, Doncaster and South Humber NHS Foundation Trust has formally launched an internal investigation.

As reported by ITV News, Ms Brookes noted that Mr Chapman had been diagnosed with schizophrenia and had suffered previous bouts of psychosis.

Senior Coroner Nicola Mundy stated that her office was awaiting a comprehensive report detailing the full extent of the injuries.

Ms Mundy added that Mr Chapman’s family found the situation “incredibly difficult” and chose not to attend the opening hearing.

His stepfather had previously travelled to the mortuary on 13 October to formally identify the body.

Coroner’s officers confirmed that the family would be kept fully updated on all future legal developments.

What Independent Inquiries Are Currently Underway?

Alongside the NHS investigation, local authority regulators have initiated their own statutory reviews.

The City of Doncaster Council, which serves as the licensing authority for Yorkshire Wildlife Park, is carrying out a health and safety investigation.

Council officials confirmed that their findings will be submitted to the coroner ahead of the full inquest.

The venue itself remained closed to the public following the incident as authorities completed initial site assessments.

A full inquest hearing has been formally scheduled for 18 March 2027 to establish the complete circumstances surrounding the tragedy.

Background of the Particular Development

The Amur tiger enclosure at Yorkshire Wildlife Park is designed to strictly meet national regulatory safety protocols for dangerous wild animals, featuring elevated perimeter fencing and security barriers.

Incidents involving visitor entry into carnivore enclosures in licensed UK zoos are exceptionally rare due to stringent licensing requirements governed by the Zoo Licensing Act 1981.

In recent years, the intersection between adult mental health care provision and emergency crisis intervention has come under increased scrutiny across South Yorkshire.

NHS mental health trusts regularly conduct thorough internal clinical reviews—known as Serious Incident Investigations—when a patient under community care dies unexpectedly, ensuring that service delivery, risk assessment procedures, and family communication protocols are rigorously evaluated.

Predictions and Potential Impact

This development is expected to have significant implications for multiple key groups:

  • Mental Health Service Users and Families: The joint NHS investigation may lead to revised protocols for monitoring high-functioning adult patients with schizophrenia who reside independently in the community, potentially changing how risk evaluations and family feedback are integrated into routine outpatient care.
  • UK Zoo and Wildlife Park Operators: Licensing authorities and industry bodies such as the British and Irish Association of Zoos and Aquariums (BIAZA) may review secondary perimeter fencing guidelines to enhance measures designed to prevent intentional unauthorized entry into dangerous animal enclosures.
  • Local Community and Visitors: Enhanced safety protocols and ongoing local authority reviews may alter visitor access procedures at regional wildlife parks to reinforce site security without compromising conservation and educational activities.
